Below is information regarding criteria for enrolling, service process, and the Horizons Centre’s application for service.
Start the application process for Persons with Developmental Disabilities (PDD)(funding) by going to PDD Intake (alberta.ca)
Setup a meeting with Horizons Centre’s Program Director By emailing Sparenteau@horizonscentre.com for a tour of the agency and information on programs and supports.
If interested, complete the Horizons Centre Application for Service.
Horizons Centre will then connect with P.D.D. to discuss eligibility, funding, and outcomes.
Upon confirmation of approval through PDD, taking into consideration the person’s PDD Outcome Plan, funding and Horizons Centre programming, the individual, their circle of support and Horizons Centre will determine a schedule and start date for services.
